Add Issue and PR Template #27

Currently we don't have any template to follow for both creating an Issue or giving a PR. I think its about time we add this in order to make it more professional.
Just the following would be enough:
For Issues :
- Bug Report
- Feature Request
For Pull Request :
Just the normal template of the contents required would work.
