Sir i want to work on this issue.Is there a way to get past medical history of the patients in the database to get insights

I have a suggestion on making at_risk group using

1. Biological situation-
   (includes-age group,sex,physiological state(like high blood pressure),health condition(like having tuberculosis in the past))

2. Physical situation-
   (includes-place of living-(urban, rural), living conditions(overcrowding)

Data I require- physiological state, health condition, place of living of the patient
Can you suggest some source.
