One common point of confusion for users is that _distance only gives the true vector distance if there is no index or if there is a refine factor that is set. If it is an indexed search with no refine factor then the _distance returned will be the distance between the quantized vectors, not the actual vectors. In fact, the vector search documents don't mention refine factor at all. However, this has a significant impact on recall. It would be good to clarify this if possible.
