A bunch of tidy up commits please pre-fill addresses on the checkout form

since CiviCRM 4.2 the class loader has taken care of require_once  (v2 api calls are an exception)
Prefills CiviCRM addresses into the checkout panes, priority is given to CiviCRM 'stock' addresses if they exist (Postal, billing) followed by UberCart once - one the basis CiviCRM ones are considered more permanent. Primary is used as a back-fill address
…ked by default - you need to add a predicate,

example usage is to add an attribute to webform allowing people to indicate they want their address updated & for a scheduled activity to be created for someone to approve the update
